Health Clinics

OFA Clinics

Eyes – CERF with Dr. Dan Ward, DVM, ACVIM – Cost is $50.00 per dog and
forms will be provided. You will need to provide your dogs registration number and
sire/dam numbers and microchip number.

Heart – OFA Cardiac with Dr. Darlene Blischok, DVM, DACVIM – Cost is $55.00
per dog. For each dog, you must complete and print the cardiac form on the OFA
website and bring it with you.

*Please note – The OFA Basic Cardiac Exam may be performed by either a practitioner
such as your regular veterinarian in general practice (designated with a “P” on your
dog’s OFA certificate) or a veterinary cardiologist who is a veterinarian with additional
training, testing and certification specializing in cardiac care (designated with a “C”
on your dog’s OFA certificate)*
